SurveyGuard provides SMS text message notifications to authorized users of the SurveyGuard platform. These messages are limited to critical task alerts related to compliance, survey readiness, and operational activity within your registered facility. This is a transactional notification service and is not used for marketing purposes.

2. Who Can Enroll

SMS notifications are available to authorized facility staff who have a registered SurveyGuard account and have provided a valid U.S. mobile phone number in their user profile. Enrollment requires explicit opt-in consent through the SurveyGuard application.

3. Message Frequency

Message frequency varies based on activity within your facility. You will only receive messages when a critical task alert is triggered in the SurveyGuard platform. SurveyGuard does not send promotional, marketing, or scheduled broadcast messages.

4. Message and Data Rates

Message and data rates may apply. SurveyGuard does not charge for SMS notifications, but your mobile carrier may charge for text messages and data usage based on your individual plan. Contact your mobile carrier for details on your plan's messaging rates.

5. How to Opt Out

You may opt out of SMS notifications at any time using either of the following methods:

• Reply STOP to any SMS message you receive from SurveyGuard. You will receive a confirmation message and no further messages will be sent.
• Disable SMS notifications in your SurveyGuard account under Notification Settings.
• Supported opt-out keywords: STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT.

7. Supported Carriers

Phone numbers collected for SMS notifications are used solely for the purpose of delivering transactional alerts within the SurveyGuard platform. SurveyGuard does not sell, share, or transfer phone numbers to third parties for marketing purposes. 9. No Guarantee of Delivery

SurveyGuard is not responsible for delayed or undelivered messages resulting from carrier outages, network conditions, device settings, or other factors outside of our control. Always log in to the platform to review your complete notification history.

10. Changes to These Terms

SurveyGuard reserves the right to modify these terms at any time. Material changes will be communicated through the platform. Continued use of SMS notifications after changes are posted constitutes acceptance of the updated terms.
